It's possible that you haven't updated it in the last 30 days. Go to the Porters' Lodge (in the Hardy Building) where there is a card reader which will update it. Once your card is working, every time you use the library entrance gates your card will be automatically updated.
If your card is not used for over 30 days and updating it at the Porters' Lodge doesn't work, it is likely that the card has become inactive. If this happens your card will need to be reactivated by a member of the Portering team.
